There are Minor Children and/or the Spouse is Pregnant

If you are married to the other parent, you will choose either the dissolution or divorce packet. If you are not married to the other parent, you will choose the custody packet.

Dissolution

If you are currently married and AGREE about ALL of the details of the following issues:

then use Dissolution Packet #1, DR-1

NOTES: Both parties must sign every page of the Dissolution Packet. If, for some reason, this is not possible in your situation, you can still get divorced by using the Divorce Complaint Packet, SHC-PAC1A.

OR

Use the Uncontested Complaint for Divorce with Children Packet, SHC-PAC9A; it is strongly suggested that you use this form if:

Uncontested Complaint for Divorce with Children

If you are currently married, have children and AGREE about ALL of the details of the following issues:

then use Uncontested Complaint for Divorce with Children Packet, SHC-PAC9A.

If you are currently married and DISAGREE about ANY of the details of the following issues:

Custody for Unmarried Parents

If you have minor children together, but are not married to each other, and need a custody and/or support order, then use:

For unmarried parents dealing with child custody, if parents AGREE about:
