Change of Name

Change of Name for Adults

An adult's name may be returned to a previous name as part of a divorce or dissolution proceeding, otherwise it must be done in a separate Change of Name Case, which you can file using the court packet Adult Change of Name Packet (CIV-698).

Packet consists of the following forms:
     Cover Sheet, CIV-698 Adobe Acrobat PDF logo

     Petition for Change of Name, CIV-700 [Fill-In PDF] Adobe Acrobat PDF logo

Change of Name for Children

A child's name can be changed as part of an adoption proceeding, otherwise it must be done in a separate Change of Name Case, which you can file using the court packet Child's Change of Name Packet (CIV-692).

Packet consists of the following forms:
