Change of Name

An adult's name may be returned to a previous name as part of a divorce or dissolution proceeding, otherwise it must be done in a separate Change of Name Case, which you can file using the court packet Adult Change of Name Packet (CIV-698).

Packet consists of the following forms:
     Cover Sheet, CIV-698
     Instructions for Adult Change of Name, CIV-699
     Petition for Change of Name, CIV-700 [Fill-In PDF]
     Application or Report of Change of Name, VS-405

A child's name can be changed as part of an adoption proceeding, otherwise it must be done in a separate Change of Name Case, which you can file using the court packet Child's Change of Name Packet (CIV-692).

Packet consists of the following forms:
     Cover Sheet, CIV-692
     Instructions for Child's Change of Name, CIV-693
     Petition to Change Child's Name, CIV-694 [Fill-In PDF]
     Parental Consent, CIV-695 [Fill-In PDF]
     Application or Report of Change of Name, VS-405
